I acknowledge that I have read or have been made aware of the safety rules pertaining to the participation in Underwater Hockey, and that I understand these rules and accept the risks which are inherent in the participation in the aforementioned sport.2. I am aware and understand that participation in the sport involves intensive physical and mental exertion. I warrant that Iam free from any medical condition that may endanger my life or well being whilst playing UWH or the life or well being of any other participant.3. I undertake not to hold the proprietor of SAUWHF liable for any injury, loss or damage which I might sustain whilst participating in the game whether in or out the pool, the complex and surrounding areas and howsoever arising irrespective of whether such loss, injury or damage can be attributed to any act or omission of an employee, manager, assistant or coach

1. All players, representatives, coaches and referees will always behave in such a way as to uphold and promote a good image of SAUWHF.2. You have read and understood the SAUWHF Code of Conduct which can be found on the SAUWHF's website.2. Any player, or representative, who fails to adhere to the SAUWHF code of conduct, may be liable to a disciplinary hearing as set out in the SAUWHF constitution.3.By participating in this event you acknowledge and agree to the SAUWHF's Anti-Doping Policy as laid out on the SAUWHF website4. SAUWHF reserves the right to refuse any team or player entry to any function held by the SAUWHF if in their opinion the player is not fit to enter.
