 1. Date/Time: Date and time is recorded using military time.

 3. How Information Was Received: Introductory statement records how the information was received - By telephone, while on patrol, etc.
 4. Chronological Order/Events: Report provides a clear picture of events in chronological order.
 5. Action Entries: Report uses Action Entries for each new paragraph. Each paragraph is started with “At HHMM Hours”, unless it references additional detail related to information in the previous paragraph.
 6. Punctuation, Spelling: No contractions, codes, slang or abbreviations are used. Information is not recorded using all CAPS. No spelling errors.
 7. Names UPPER CASE: Last names are UPPER CASE.
 8. Description of Events: Report provides a clear picture of events in chronological order including participation of all parties involved.
 9. Use of Acronyms: Acronyms are used after initial reference. Acronyms for building or location are recorded for later reference. Example: World Wide Headquarters (WWHQ).
 10. Arrival/Departure/Identity of All Parties Involved: All parties are identified including title and role in the activity. Police, Ambulance or other outside parties are identified by name of city or name of company.
Description: Deere Employee reported experiencing chest pains. Factory Nurse and local EMS were dispatched. Employee was transported to General Hospital without further incident.
