This example shows that how to capture information about the quiz taker. The below form is an example. You can ask any custom question you need such as ID Number, class, department etc. You can add both type in and "select from list" style questions to capture information about quiz taker. This information is stored & made available to quiz author with the detailed report of the quiz taker so you can review who took the quiz.
