Reading interest survey

This is a reading interest survey, whose target audience is primarily sophomore and junior high school science students.  The results of this survey will allow me insight as to the initial attitudes of my students and will go into that student’s learning portfolio for the year.  At the end of the year, this same test can be taken again and compared to the earlier one to see if the student has experienced a change in their opinions.
