Activity Purpose and/or Learning Outcomes
  1. The learner will be able to define Medical Error.
  2. The learner will be able to have an awareness of the socioeconomic aspects of medication errors. 
  3. The learner will be able to recall the five (5) main phases of the medication process and the phase in which the highest rates of error occur.
  4. The learner will be able to name the five (5) main causes of medication errors as described by the National Coordinating Council for Medication Error Reporting and Prevention Taxonomy of Medication Errors. 
  5. The learner will be able to understand the concepts of ‘active failure’ and ‘latent condition’ as it pertains to the causes of medical errors.
  6. The learner will be able to recall four (4) environmental factors that contribute to medication errors; identify four (4) sources of noise in the hospital setting; recall the acceptable noise level as determined by World Health Organization (WHO); understand the correlation between light levels and medication errors. 
  7. The learner will be able to list the high-alert medications, as determined by the Institute for Safe Medication Practices.
  8. The learner will be able to list the seven (7) rights method.
  9. The learner will be able to understand the rationale of reporting medication errors.
