Summary of Session Goals:
To introduce the audience to the AHRQ Guide to Improving Patient Safety in Primary Care Settings by Engaging Patients and Families, and specifically the Warm Handoff Plus intervention. We will describe the goals of the Guide and the Warm Handoff Plus intervention.
