To introduce the audience to the AHRQ Guide to Improving Patient Safety in Primary Care Settings by Engaging Patients and Families. We will describe the goals of the Guide and the Create a Safe Medicine List Together intervention, and walk through the materials developed to support adoption of the Medication Management strategy in practice.
