A work package that does not meet the requirements, but is later deemed acceptable by the customer-monitoring & controlling - quality managementremember a defect is any deliverable that does not meet the documented requirements. choice a clearly is the only choice that matches this description. just because a customer later decides that it is acceptable does not negate the fact that it did not meet the requirements and thus is a defect. in choice c, even if the deliverable doesnt work properly, if it meets the requirements then it is not defective. change requests are not necessarily derived from defects.
