Which laboratory value should be closely monitored?

A client with renal insufficiency is admitted to the hospital with pneumonia. He's being treated with gentamicin (Garamycin).

Corrected answer is option A – Blood Urea Nitrogen (BUN)

A client with renal insufficiency on antibiotics should have the BUN and creatinine closely monitored. Aminoglycosides such as gentamycin are known to be nephrotoxic. To know the state of the kidneys the BUN and creatinine should be closely monitored.

Sodium level is one of the laboratory values routinely monitored in patients that have been hospitalized.

Alkaline phosphatase is measured to evaluate the liver and not the kidneys.

WBC can be measured in this client but it will only tell how the white blood cells are responding to the infection not on the state of the kidneys, therefore, there is no need to closely monitor it.

Blood urea nitrogen (bun). Bun and creatinine levels should be closely monitored to detect elevations caused by nephrotoxicity. Sodium level should be routinely monitored in all hospitalized clients. Alkaline phosphatase helps evaluate liver function. The WBC count should be monitored to evaluate the effectiveness of the antibiotic; Itdoesn't help evaluate kidney function.
