Nurses are required to notify the physicians if anything unusual happens in certain situations. For example, if a nurse is conducting her or his normal routine with a patient whose routine is to get out of bed for breakfast which he normally does, but today is different.

Today, the client feels dizzy when he gets out of bed, this would be a cause for alarm since this normally doesn’t happen and the medication hasn’t been changed.

If the client takes his medication during breakfast, then the nurse should not give the medication, but instead notify the physician to see if there is a problem with the client. The physician will know what to do and then the nurse will not have to deal with the medication already being inside the patient’s system.
