At the beginning of a patient’s visit to a hospital, there are several things that must be done during the initial care plan. This occurs after the nurse has already assessed and diagnosed the patient.

For example, if a patient is admitted to the hospital because he is suffering from a cough. He tells the nurse that he is having night sweats and a fever, she will need to implement a plan. She may realize that he has a respiratory problem.

Therefore she will place the patient in the respiratory isolation. This means that the patient could be contagious and he could also worsen if exposed to other germs. The nurse must wear a mask to enter the room where this patient is staying.

Placing the client in respiratory isolation because the clients signs and symptoms suggest a respiratory infection (possibly tuberculosis), respiratory isolation is indicated. Every 8 hours isnt frequent enough to assess the temperature of a client with a fever. Monitoring fluid intake and output may be required, but the client should first be placed in isolation. The nurse should wear gloves only for contact with mucous membranes, broken skin, blood, and other body fluids and substances.
