You have completed the self check tool and attend an investigation meeting to give more information-2. provide evidence to back up your allegations-3. act as a witness during a disciplinary hearing-the operational risk manager will start the investigation process which involves many very important steps. as part of the investigation they will want to speak to you formally about the allegations you have made and ask you to attend an investigation meeting. you will be asked to bring along any evidence to support what you know and back up your allegations. however you will not be asked to produce an investigation report. once the investigation meeting has taken place, it is the job of the operational risk manager to pull together a report which will be shared with a senior manager who will make the decision whether to call the person the allegation was made about to a disciplinary hearing. you will be asked to attend the hearing to state your evidence again to the senior manager.
