The best option for this is letter D. if the other options would be considered, placing the tracheostomy tube will only delay what should be done. By doing letter D, it will be easier for the patient to breathe. The tube can be re-checked later on by the physician or the respiratory therapist.

Doing letter B is not going to be ideal as this will block the airway of the patient. This may result in the patient having more trouble breathing. Take note that this is an emergency situation that would require the nurse to act fast. The way that a nurse reacts will have an effect on the patient’s overall condition.

Grasp the retention sutures to spread the opening.

If the tube is dislodged accidentally. the initial nursing action is to grasp the retention sutures and spread the opening.Options A and C: Calling respiratory therapist or the physician will delay treatment in this emergency situation.Option B: Covering the tracheostomy site will block the airway.
