The correct answer is option B
If there is a problem with fluid volume, restriction of sodium in the diet of the client should be included in the plan care.

Fluid volume excess refers to expansion of extracellular fluid compartment. Sodium causes water retention.

Therefore, it should be decrease to assist with the process of volume loss from the body.

Option A is incorrect because the nurse is not expected to change the order given by the physician.

Option C is wrong because elevated blood glucose is known to cause volume constriction and not expansion.

Monitoring it will not do much for this client.

Haemodialysis may be useful if the fluid volume expansion was as a result of renal failure but this question doesn’t provide further details on this.
