Why is Nancy's assessment inaccurate?
Nancy has to do an assessment on Mrs. Jones. Nancy has a limited amount of time to do her assessment. She quickly takes her assessment on Mrs. Jones who was just admitted with a fractured hip. Mrs. Jones states her pain is 9 out of 10. Mrs. Jones is also worried that her little dog at home is going to starve. Nancy listens while she does her assessment and feels proud that she got her assessment done in 10 minutes.

Why is Nancy's assessment inaccurate?

1. Mrs. Jones is an older patient who is obviously confused. The EMT said nothing about a dog.
2. Mrs. Jones was in a lot of pain.
3. Nancy was distracted and in a hurry.
4. Mrs. Jones was distracted about her dog.
5. Nancy shouldn t have listened while assessing.

Nancy could pay attention on Mrs. Jones pain, Nancy shoulsd asked questions after mrs. Jones medication had set in.

Mrs. jones was in a lot of pain.-2. nancy was distracted and in a hurry.-3. mrs. jones was distracted about her dog.-a patient in a lot of pain is probably not going to give exact data and pain management should be the first concern. mrs. jones, in a lot of pain, was concerned about her dog and distracted. through the pain and her worry, could she have not been paying attention to nancy when nancy asked questions? nancy was rushing to get her assessment because of the limited amount of time. maybe the assessment could have been rescheduled until after mrs. jones pain medications had set in and nancy had more time?
