What is the difference between EHR and PHR?

EHR is the short form of Electronic Health Record, while PHR is the short form of Personal Health Record. From their full forms, it is very easy to figure out that both are not referring to the same thing. EHR is used by medical practitioners in the hospital for data collection. In contrast, PHR is a personal health record of an individual. Unlike in PHR, where the data can be documented either manually or electronically, the data in EHR is documented electronically.

EHR contains different data or information from different health providers, whereas the PHR contains certain information about an individual. Unlike EHR, which is created by the health practitioners, PHR is created by the user. A typical PHR usually reflects the special diet, medicine administered, symptoms of a particular disease, exercise programs, etc. In contrast, EHR contains comprehensive information about someone's health condition. With PHR, an individual can monitor their health, but EHR gives a medical practitioner a better understanding of the previous health issues of a patient so that better treatment can be provided.

EHR stands for Electric Health Record, and PHR stands for Personal Health Record. In EHR, the hospital or health care center has authority over the documents. However, in PHR, it is the individual who maintains control over them. In EHR, all of the information is documented electronically. On the other hand, it is manually recorded in PHR.

EHR is the total data of patients from all of the providers they have had. It is a culmination of information that was created by these health practitioners. PHR, on the other hand, is only pertaining to the information needs of the person created by them. A PHR contains useful information concerning the symptoms if the medicine is taken, allergies, and illnesses.
