You must have or be suffering from gender dysphoria
You must have lived for at least two years as the preferred gender
You must have evidence that you have lived for two years or more as the preferred gender
You must be single
You intend to live permanently in their acquired gender

Whilst having had surgery or waiting for surgery counts as evidence for your case of gender transference it is not required; however, if you do not want to have surgery you will be expected to explain why. You do need two medical reports however only one is from the GP, the other should come from your gender specialist. You must also be 18+ not 21+
