What conditions would stop a phlebotomist from drawing blood from a patients vein?

Phlebotomy should not be done on the same side with mastectomy.

Why would it be the IV in the opposite arm if you cannot draw blood from the same side of a woman's mastectomy. You can draw on someone with an IV as long as the vein is above the IV, besides that there would be no negative effect if you draw from the opposite arm.
