Validation works in such a way that the person who received the signal will reciprocate or will show that he has understood the signal quite well. For example, a patient has a weak right leg because she broke it recently. The patient should not put a lot of pressure on that side of their body. Instead, they should have support like a crutch.

If the person is in the hospital and they do not have crutches because they need help going to the bathroom, then the nurse should support that person on their right side so they do not have to put pressure on their right leg. Sometimes, people may lean on the helper or aide for much-added support.

What about the respirations of 9 per minute? Isnt that a concern? If the patient urinated 20 ml per hour for the 2 hours why would it be a concern if the patient has 8 hours/shift to be observed for urine output.
