The posterior pituitary is an extension of the hypothalamus.

The hypothalamus produces neurohormones ADH and Oxytocin, which are stored in the posterior pituitary. When triggered by the hypothalamus they are released.
How does the hypothalamus work together with the anterior pituitary gland?
 
The anterior pituitary is triggered by releasing hormones from the hypothalamus to either secrete or inhibit the production of its hormones.

The portal blood vessels is the direct connection between hypothalamus and anterior pituitary by way of capillary beds in both structures.
