What are 4 causes and clinical conditions that might cause a pos DAT in both infants and adults?
HDN, autoimmune hemolytic anemia, red blood cell sensitization caused by drugs, and transfusion reactions(septicemia, bacterial contamination).
What are the 2 methods of elution?
Lui Freeze Thaw and Acid elution
When is the Lui Freeze Thaw elution method used?
It is used for suspected ABO incompatibility that causes ABO antibodies to stick to the cells.Usually done on babies with pos DAT due to ABO abs.It is faster and simpler than an acid eluate.
When is the acid elution method used?
To detect all other suspected IgG antibodies causing a pos DAT.
What clinical info might help to determine the cause of a positive DAT?
The pt's diagnosis, drug therapy, recent transfusion history(maybe given ABO incompatible units), evidence of in vivo hemolysis, unexpected antibodies in pt serum, if pt is receiving IVIG or IV RhIG, and did they receive equine preps of antilymphocyte globulin and antithymocyte globulin.
