Respiratory acidosis
Correct answer: a. Headache, tachycardia, restlessness, and confusion are S&S of respiratory acidosis for which a client with COPD is at risk. Other S&S include dyspnea, tachypnea, diaphoresis, and anxiety progressing to lethargy, ventricular dysrhythmia, dilated facial and conjunctival blood vessels, cyanosis, and coma.
