It's called PHR whichstands for Personal Health Record. It will provide all of the data and information that will be very helpful for the patient so that proper care can be given. The main reason why this exists is to that it will be easy to acquire the complete data of the patient especially when it is needed.

This can be transferred electronically. For example, if the patient is hospitalized in a different state from his home state, the data can immediately be transferred. This will help the attending physician know more about the person’s potential condition. The data that will be written here will be accurate as it can potentially save someone’s life.
