While you are at the DOS prompt, place a diskette into the floppy disk drive and type format a: and see if you are getting any errors like the following:
Invalid media or Track 0 Bad: Try to format another floppy disk.
Invalid drive specification: Ensure that your floppy drive is setup properly in CMOS.
If the floppy disk drive formats properly in MS-DOS but does not format in Windows there is a possibility that Windows or a program within Windows is obstructing the floppy drive from working. End the task. Still if you continue to experience the same problems it is recommended that Windows be reinstalled.

Bad floppy drive.
Even after following the above steps, if you still continue to experience problems it is very much likely that the hardware within the computer is bad. Try to replace the following hardware in your computer in the below mentioned order.
Replace the floppy data cable that connects the computer floppy drive to the motherboard / I/O board.
Replace the floppy if the floppy data cable did not resolve the issues.
Replace the motherboard / I/O board.
