Not connected properly.
Open the computer.
Then check to see that the floppy drive is connected to the motherboards FDD connector. If it appears to be connected, disconnect and reconnect the cable to ensure the cable is seated properly.
See if the floppy cable coming from the motherboard is connected to the back of the floppy drive properly. If it is connected, disconnect and connect the floppy drive cable once again to verify that it is seated properly.
Ensure that a power connection is also given to the back of the floppy drive.
Bad drivers.
If you are unable to read or write from / to a floppy diskette from Windows, ensure that the computer is not displaying floppy driver conflict by testing the floppy drive from MS-DOS.
In order to get to MS-DOS prompt while running Windows 3.x, click File and click Exit Windows.
